Phase II Cardiac Rehab

Phase II Cardiac Rehab is for patients immediately after discharge from the hospital who experienced a heart attack, coronary artery bypass graft, patients with stable chest pain, peripheral artery disease, stent placement, and other cardiovascular diseases.
