Grinnell Regional Hospice
Admission into the Grinnell Regional Hospice requires a referral by an individual who recognizes a need for the services. A physician referral to hospice is not required, but anyone who understands the needs of the patient including family members, physician, neighbors, and clergy may refer a patient. Once a referral is made, a hospice staff member will contact the patient and gather pertinent information on the diagnosis as well as demographic information. This information is transferred to the medical director of hospice who works with the patient’s family physician to confirm that the diagnosis is terminal. Persons may be admitted to Grinnell Regional Hospice if they have been diagnosed with a terminal illness with a life expectancy of six months or less. If all criteria are reviewed and approved for admission, a hospice team will meet with the patient and begin coordinating necessary services for the patient.
To speak with someone in the hospice admission area, e-mail or call 641-236-2418.
