You can receive rehabilitation services like physical and occupational therapy, dental care, hearing aid services, and other important diagnostic screenings and services in Toledo.

Deer Creek Health Center has been labeled a new model for rural healthcare. It brings together neighboring hospitals to provide services to you. The health center is a partnership between Grinnell Regional Medical Center and University of Iowa Health Care. This unique partnership allows you to have greater access to services, physicians, and specialists at this one-of-a-kind healthcare facility.

Deer Creek Family Care
The physicians and staff at Deer Creek Family Care serve your community and provide primary care for your entire family, from the youngest member to the oldest.

Services offered through Deer Creek Family Care include general medicine, women’s health, health promotion, health screenings for adults, and preventive healthcare.

Healthcare professionals available to care for you are:

  • Sherry L. Parks, PA-C, joined the Deer Creek medical team in 2001. She received her physician assistant degree from Des Moines University Osteopathic Medical Center in 2000. She is also an emergency medical technician for the Garwin ambulance service.
  • Scott J. Piper, MD, received both his undergraduate degree in biomedical engineering and his medical degree from the University of Iowa.  His medical residency was at St. Mary’s Family Practice Residency program in Evansville, Ind.

Deer Creek Family Care hours are 8 a.m. to 5 p.m. Monday through Friday. This clinic is a joint venture between Grinnell Regional Medical Center and University of Iowa Health Care.

Deer Creek Dental Care
Let this full service dental office meet all your family dental needs. Deer Creek Dental Care offers general dental services, including bleaching, crowns, and restorative care. It does not offer orthodontia services.

Deer Creek Dental Care is the private practice of Karen Brooks, DDS. She received her dental degree from Howard University in Washington, D.C. She has been in dental practice for 20 years, including 10 years in Detroit and five years at the Plaza Dental in Toledo before opening Deer Creek Dental Care in May 1999.

Deer Creek Dental Care hours are 8:30 a.m. to 5 p.m. Monday through Thursday.

Deer Creek Sleep Lab
Deer Creek Health Center and GRMC offer sleep studies at a new sleep lab in Toledo. This collaboration with Michael M. McCubbin, MD, of Sleep and Pulmonary Services, Inc., allows area residents to have a sleep study. This overnight test evaluates how well you sleep and determines any potential problems preventing you from getting a good night’s sleep. It evaluates a person’s sleeping patterns and monitors signals from the patient’s brain, heart, muscle, eyes, and breathing. This information helps identify sleep disorders.

Common sleep disorders are:

  • obstructive sleep apnea
  • restless legs syndrome
  • insomnia
  • narcolepsy

A sleep test can help diagnosis these conditions so medical or behavioral changes can be made.

Signs of a sleep disorder are:

  • loud snoring
  • gasping
  • choking
  • regular coughing during the night
  • pauses in breathing while sleeping
  • excessive daytime sleepiness (trouble staying awake watching TV, driving, or working)
  • poor judgment or concentration
  • increasing memory loss
  • high blood pressure
  • depression
  • family history of obstructive sleep apnea
  • morning headaches
  • large neck size (>17” in men, >16” in women)
  • obesity
  • irritability
  • attention deficit hyperactivity disorder  (applicable for children)

Individuals with any of these symptoms should speak with their healthcare provider for diagnosis. Area family practice physicians may refer patients to the Deer Creek Sleep Lab. The Deer Creek Sleep Lab brings this much needed diagnostic tool closer to home.

Each test takes approximately eight hours. Usually a minimum of six hours of sleep data is needed for insurance purposes and a patient should anticipate spending eight hours at the sleep test.

Sleep rooms resemble a hotel or a person’s own bedroom. We have full and queen size beds, regular linens (not hospital sheets), and an inviting environment. We make every effort to ensure that each individual is comfortable.

Deer Creek Wellness and Rehab Center

The Deer Creek Wellness and Rehab Center offers physical therapy, occupational therapy, and dietary education classes to residents in the area. Be assured that when your physician refers you to one of these services, you will find excellent providers and helpful educators at the center.

Physical Therapy
Physical therapy services help patients suffering muscular and skelatal injuries and conditions, from chronic pain to orthopedic surgery recovery. Physical therapy offers a wide range of treatments and services that further rehabilitate or help restore strength, mobility, flexibility, and pain control. Depending on your individual needs, the physical therapist will work with you to develop a personal program to best meet your goals.

The Deer Creek Wellness and Rehab Center is open from 8 a.m. to 5 p.m., Monday through Friday.

Occupational Therapy
Occupational therapy services help patients improve their ability to do the tasks of work, leisure, social interaction, and daily living when illness, disease, developmental conditions, and injury interfere with daily tasks. The occupational therapy staff work closely with physicians, nurses, and other healthcare professionals to provide you with a customized treatment program aimed at improving abilities.

Specialists seeing patients at Deer Creek Health Center:

  • Orthopedics – Douglas Cooper, MD
    Cooper received his B.S. and medical degree from the University of Iowa. He completed his orthopedic residency at the University of Iowa. He specializes in the diagnosis and treatment of disorders, diseases, and injuries of the skeletal system and associated motor organs.
  • Neurology (Grinnell Neurology) – Michael J. Rosenfeld, MD
    Rosenfeld received a medical degree from the University of Connecticut School of Medicine, Farmington, Conn. His training includes a movement disorders fellowship from William Beaumont Hospital and Clinical Neuroscience Center, in Southfield, Mich. His residency was at Brown University School of Medicine/Rhode Island Hospital, Providence, R.I. He specializes in disorders of the nervous system - brain, spinal cord, nerves, and muscles.
  • Podiatry – Matthew McKnight, DPM
    McKnight received his doctor of podiatric medicine from the Des Moines University Osteopathic Medical Center. He completed a podiatric surgical residency at the Forest Park Hospital in St. Louis. He specializes in foot and lower extremity diseases and conditions.
  • Surgery – David S. Kermode, DO, FACOS
    Kermode received his medical degree from Kirksville College of Osteopathic Medicine, Kirksville, MO. He then completed a medical internship at the Naval Hospital in Jacksonville, FL. After completing his internship he spent a year as the Medical Officer for the Naval Support Force in Antarctica. Kermode then completed his general surgery residency at the Naval Hospital in Corpus Christi, Texas, where he was the head of the general surgery department. He specializes in general surgery, including vascular surgery, breast surgery, thyroid surgery, and the full range of abdominal procedures.

Technology advances continue to make highly sensitive, high-precision equipment mobile and Deer Creek Health Center is taking advantage of the trend. Mobile diagnostic services visit the health center on a regular schedule. Bringing the technology to you allows you to stay close to home while receiving these important services.

Mobile services describe a category of diagnostic services that are placed in a semi-trailer/truck and driven from community to community.

Diagnostic services available at Deer Creek are mammography, echocardiography, ultrasound, MRI, and bone densitometry. Your family physician will refer you to any of these services as needed.

On-Site Services
Deer Creek Health Center houses a laboratory for routine lab tests and a radiology suite for x-ray services that assist healthcare providers in diagnosing your healthcare concerns. A procedural room is also located in the facility to accommodate medical conditions that require a large work area, such as stitching lacerations, taking EKGs, and setting broken bones.
